1095-C Tax Form Information

Dear GW Faculty and Staff,
Form 1095-C is a tax document that reports information about your medical coverage in 2025. While you will not need to include your 1095-C with your 2025 tax return filing, or send it to the IRS, you may need information from your 1095-C to help complete your tax return. Think of it as your “proof of medical insurance” for the IRS.

Who receives a 1095-C form?
Any GW employee who was considered full-time at any point in 2025 or who was enrolled in a GW medical plan in 2025. The Affordable Care Act defines “full-time” as an employee who worked an average of 30 or more hours per week.

Electronic Access for your 1095-C form*
1095-Cs are now accessible online via the Benefits Enrollment System. Follow the steps below to review, download, and print your 2025 form:

Once you are logged into the system, select Manage Profile. This option is found by clicking the down arrow next to your name in the upper right hand corner.
- Locate the section called Manage Documents. Click View All Documents.
- Scroll down to find 1095-C Form 2025 in the description column, and click View in the last column on the right to download your form.

*Please note: If you did not consent to receive your 1095-C electronically, you will also receive a paper version through regular mail on or before March 2, 2026. If you would like to receive your 1095-C only electronically next year, please visit the Benefits Enrollment System and select 1095-C Electronic Consent under Find It Fast. Scroll down and click edit.

There are three parts to the form:

- Part 1: reports information about you and your employer.

Part 2: reports information about the coverage offered to you by GW, the affordability of the coverage offered, and the reason why you were or were not offered coverage.
- Part 3: reports information about the individuals (including dependents) covered under the self-insured plan.
