eBook: Social Determinants of Health and Health Disparities: Taking Action

PHA’s  Quality and Research Committee led by Dr. Mary Jane Osmick completed a very important White Paper - Taking Action on Social Determinants of Health to Improve Outcomes. This research found that SDH are modifiable factors that hamper efforts to achieve improved health outcomes, often resulting in health disparities within populations. SDH include the domains of:
  • Economic stability (including employment status, level of education, housing stability, physical environment, personal and community safety, food stability, lack of reliable transportation)
  • Access to quality health care including both physical and behavioral/mental health resources and access to affordable health insurance
  • Negative and positive health behaviors (attitudes and actions by the individual in relationship to health outcomes)

ICYMI - PHA Provider Series Webinar: Evaluating the Impact of Year-Long, Augmented Diabetes Self-Management Support 

The American Association of Diabetes Educators (AADE) conducted the randomized controlled study titled “Evaluating the Impact of Year-long, Augmented Diabetes Self-Management Support” with individuals who had a diagnosis of type 2 diabetes in underserved populations. AADE compared Hemoglobin A1C and other clinical and behavioral outcomes of the over 400 participants who were randomized to receive Diabetes Self-management Education (DSME) with one year of additional telephonic support (intervention group) versus those randomized to receive DSME alone (control group). The results were interesting.

GEHA Sponsors Heartland Honor Flight for Second Year

For the second year, GEHA (Government Employees Health Association) is a sponsor for Heartland Honor Flight, assisting the organization in giving more than 80 veterans of World War II, Korea and Vietnam a full-day trip to Washington, D.C. to tour the memorials honoring their military service. The trip occurred on April 30, at no cost to the veterans. GEHA’s financial support helps to offset the cost of the chartered flight, box lunches and leasing tour buses for the veterans. More

The 2019 IBM Watson Health 15 Top Health Systems study recognizes five large, five medium and five small systems from 337 health systems and 2,961 hospitals across the U.S. More


Wellsource Health Risk Assessment Data Reveals Link Between Happiness, Good Habits and Health

Good lifestyle habits are positively associated with happiness and mental health, according to data from Wellsource, a premier provider of evidence-based, NCQA-certified Health Risk Assessments (HRA) and self-management tools. The company examined self-reported HRA data from more than 270,000 individuals from the ages of 18 to 85 to see what influences mental health, specifically happiness. Their published data review provides recommendations for population health professionals to consider for improving happiness and lessening psychological distress, which can lead to a more productive workforce. More


The Care Center Collaborative to Partner on Value-Based Care Contracting

Through their subsidiary organizations, the Medical Society of Delaware (MSD) and the Pennsylvania Medical Society (PAMED) have agreed to join forces to explore development of a joint approach to pursue value-based care arrangements for physicians in their respective states.  MSD’s subsidiary, the Medical Network Management Services of Delaware, LLC (MedNet) and PAMED’s subsidiary, The Care Centered Collaborative will collectively identify a pathway for the development of a unique regional enterprise dedicated to improving access to quality care, reducing costs, and improving both patient satisfaction and physician satisfaction in both states.  More


Solera Health Adds A Diverse Group of Established Business Leaders to Company Advisory Board
