"Chemotherapy remains a cornerstone of curative intent treatment for early‑stage breast cancer, but its prognostic benefit depends on patients receiving the planned dose and schedule. Serious adverse events can accompany chemotherapy and frequently lead to dose reductions, delays, or early discontinuation. Relative dose intensity (RDI)—the proportion of chemotherapy actually delivered relative to the planned regimen, accounting for reductions and delays—is therefore a critical indicator of treatment tolerability and adherence. Achieving ≥85% RDI is widely considered necessary for optimal clinical benefits. However, while clinical and treatment‑related predictors of suboptimal RDI are well described, the influence of modifiable lifestyle factors remains poorly understood.
The Alberta Moving Beyond Breast Cancer (AMBER) Cohort directly measured cardiorespiratory fitness, muscular strength and endurance, body composition, physical activity, sedentary behaviour shortly after breast cancer diagnosis in 890 patients who received (neo)adjuvant chemotherapy. Overall, 81.6% achieved ≥85% RDI. Higher aerobic fitness, greater muscular strength, and a healthier body composition (i.e., higher lean mass and lower adiposity) were associated with achieving ≥85% RDI. These results shift the focus beyond static clinical factors, highlighting key, modifiable determinants of chemotherapy tolerance in future research." - Renee Kokts-Porieti, MS

"People with metastatic breast cancer often experience a loss of muscle mass, muscle strength and physical performance. Research has shown that this can make treatments harder to tolerate and may lead to poorer health outcomes and quality of life. Whilst there is strong evidence that structured exercise can improve muscle mass, muscle strength and physical function in people with localised breast cancer, we were interested to see whether this also applies to people with metastatic disease, who often receive ongoing treatments and experience more severe symptom burden. As part of the multinational PREFERABLE-EFFECT study, involving 357 participants, we assessed body composition, lower body strength, handgrip strength and functional performance and were able to demonstrate that indeed participants who were randomised to a 9-month supervised exercise program, involving aerobic, resistance and balance exercises, gained more muscle mass and became stronger, especially in their lower body, compared to those who did not receive a structured exercise program. They also showed improvements in their physical abilities such as balance and overall function. These results further support our previously reported study findings that supervised exercise can be a safe and effective way to help people with metastatic breast cancer maintain or improve their health and wellbeing." - Eva Zopf, PhD
