The First Report of Incident is now electronic!  RMS is excited to launch the digital version of the First Report of Incident. Supervisors should use this form to report all injuries to Risk Management & Safety.  This form can be accessed on RMS's Worker's Compensation page. 
Radon Gas

Radon gas is a naturally occurring radioactive gas that has, in large concentrations, been linked to an increased risk of lung cancer.  Risk Management & Safety has tested many of the University's buildings for radon over the past 20 years, and has not seen anything exceeding the EPA Action Level of 4.0 pCi/liter.  If you would like to know the results of a study in a specific building, or would like a test/retest conducted, please contact us at  Areas most likely to have an elevated level are basements or ground floors.

At Notre Dame:
Needle Stick
An employee received a needle stick from a hypodermic needle while repairing a sink drain stopper.  After reaching into the sink to hold/remove a stopper, the employee was pricked by a needle that was beneath the sink stopper.  For more information see Needle Stick.
Elevator Laceration
An employee was injured when their hand was caught between the closing doors of a service elevator resulting in a laceration that required over 30 stitches. For more information, see Elevator Laceration.
Thumb Tip Amputation
An employee's right thumb tip was amputated while attempting to perform routine preventive maintenance on a return fan.  The return fan had been de-energized, but was "free wheeling" due to building air flow as the supply had not been de-energized.  While removing the fan guard, the employee's glove was pulled into the rotating flywheel amputating the right thumb tip without bone loss.  The employee was treated at a local hospital. At Other Universities:
Battery Fire - Boise State

Lithium batteries left unattended while charging caused a fire at Boise State University. Lithium batteries can be hazardous when they are overheated or overcharged and should be stored away from combustible materials. For more information see Fire at BSU.
For a work related injury at the University, name two common medical providers besides the Wellness Center and Pharmacy.

Congratulations to Lisa Yates, the winner of our September Trivia Contest  She defined flashpoint correctly with the following:  "The minimum temperature at which a liquid or a solid emits vapor sufficient to form an ignitable mixture with air near the surface of the liquid or the solid."
Contaminated Wipes
Wipes contaminated with solvents or oils must be managed by University personnel per Indiana Department of Environmental Management (IDEM) requirements. IDEM considers wipes to include:  a shop towel, rag pad, or swab made of wood pulp, fabric, cotton polyester blends, or other materials; gloves and clothing are also considered under this rule.  These contaminated wipes can either be laundered by RMS approved vendors or picked up for disposal by RMS.  Wipes must be contained in closed drums with labeling that says "Solvent Contaminated Wipes" or "Used Oil-Contaminated Wipes" depending on the drum's content. Storage of these wipes in drums on campus shall not exceed 180 days.  Please contact Jason DeWispeleare with questions at
