A Message from the Surgeon Chief


Dear colleagues,
We are either blessed or cursed with the complexity of our human operating systems. The Reticular Activating System (RAS) wakes up in the morning and begins to filter through billions of bits of information to support it’s primary “convictions,” setting the tone for the rest of our body. The optimist is counting blessings and identifying all the ways they are succeeding, maybe even THRIVING… even against the odds. The optimist is motivated to achieve that next level. There is a cumulative effect of prayer, affirmation, meditation, or however you choose cultivate the confidence, sense of well-being, and faith that every uncomfortable moment is only passing through. Your purpose is evidenced by the divine work that you do here. We can choose to be open, curious, optimistic, and hopeful. We can always take the high road. How do we do this as a collective? We focus on the mission: the children we serve. We have a front row seat to observe as they meet remarkable challenges without the disabling fear of knowing too much. We have the privilege of guiding them through. We are the light in the dark. This is why we show up.

Warhoover to Assume Presidency of Pediatric Orthopaedic Practitioners Society (POPS) During Annual Meeting in Nashville on April 25; Belcher to Present


The Pediatric Orthopaedic Practitioners Society (POPS) will hold their Annual Meeting in Nashville in conjunction with POSNA on Tuesday April 25. Tracy Warhoover, MSN, APRN, CPNP is the Academic Program Chairperson and President elect of the Society and will become president during the transfer ceremony at the meeting. Lydia Belcher, APRN, CPNP-AC is presenting an invited talk entitled “Diagnoisis and Treatment of Developmental Dysplasia of the Hip.”

Spotlight on Academia


Neurosurgery:
Vanderbilt is leading a multicenter, international study including >30 centers from more than a dozen countries to accumulate sufficient study subjects to be able to understand this unique disease.  Disseminated low-grade gliomas are rare but challenging tumors with increased morbidity and mortality relative to other low-grade pediatric brain tumors.  We believe there are molecular alterations that drive dissemination/metastasis of these otherwise benign-appearing tumors.  Due to small numbers, single center studies fall short in providing clinically relevant results. As of February 2023, all Vanderbilt tumors have been submitted and are undergoing Next Generation Sequencing. Read more here.

Recruitment continues for two active prospective studies to develop non-invasive biomarkers of brain development/ICP in craniosynostsosis
Left/craniofacial registry now over 1200 entries. 
Standardized facial photography app in Haiku has been accepted for innovative talks at three separate plastic surgery meetings this spring.

The abstract titled Utilizing EMR Features to Detect High-Risk Surgical Patients, has been accepted as a Magnet poster presentation for the 2023 ANCC National Magnet Conference® and Pathway to Excellence® Conference.

INNOVATION

Cleopatra: A Hands-Free Recording Device for Surgical Training, Education, Publication


Cleopatra is a video recording device designed to provide a safe, hands-free way to film your open procedures for education, research, and/or publication. It was designed and built by one of our own VUH surgeons and academic researchers, Dr. Alexander Langerman, and he is sharing with us a beta prototype for our use. Dr. Langerman is studying “surgical transparency” – how to enhance the transfer of information into and out of the operating room for the benefit of patients, hospital staff, and the healthcare system. As part of the Cleopatra effort, Dr. Langerman is developing a deidentified video database protocol to allow safe storing of clinical video for educational and research use. Dr. Irving Zamora will act as Principal Investigator responsible for the Children’s Hospital side of this database. As of now, the database itself is still being developed, so surgeons should continue to use appropriate privacy practices for any videos they record.

New Multidisciplinary Ortho-Rehab Clinic


The multidisciplinary team of medical and surgical spasticity specialists are excited to announce the start of a second multidisciplinary clinic. The new clinic is an Ortho-Rehab Clinic formed through collaboration between Orthopedic Surgery and Physical Medicine and Rehabilitation. This clinic builds on a growing program that is anchored in the larger Multidisciplinary Spasticity Clinic, which is staffed by Developmental Pediatrics, Neurology, Physical Medicine and Rehabilitation, Neurosurgery, and Orthopedic Surgery. Over the last year through contributions of Dr. Nate Lempert, the group realized that many patients’ needs could be better addressed through this new focused multidisciplinary Ortho-Rehab Clinic.  The team also adds Dr. Craig Louer of Orthopedic Surgery to the team to help meet all of the orthopedic needs of these children. From the Physical Medicine and Rehabilitation group, Drs. Elizabeth Martin and Elise McClanahan complete the team. 
Getting a Makeover: Monroe Carell ORs


It’s been 20 years since the operating rooms at Monroe Carell Jr. Children’s Hospital were first built. More than 18,000 surgeries are performed by Monroe Carell surgery teams every year.

What’s New: Monroe Carell has begun the first major renovation of it ORs since the hospital opened in February 2004. Each OR will take about 13 weeks to complete. The initial phase of renovations will overhaul 10 of the hospital’s 18 ORs, focusing on two surgical suites.The first two rooms to undergo renovations will be the cardiac room and a multiuse room used primarily for robotic surgery cases.

Surgical Quality Council Implements Repository for Surgical Adverse Events and Morbidity & Mortality 


The Surgical Quality Council, in partnership with PM&I, is working to implement a Surgical Adverse Events and Morbidity & Mortality Repository. This program is designed to systematically identify and capture all possible opportunities for improvement across surgical service lines. An M&M Structured Reporting form is in place to be utilized by all surgical subspecialties for reporting and tracking of corrective actions, as well as a dashboard for monitoring OR Returns and Readmissions. This data analytics infrastructure will support the work of the Council and establish the foundation to pursue ACS Level I Children's Surgery Verification. 
The Children’s Surgery Verification (CSV) Quality Improvement Program, commonly referred to as the CSV Program, specifically addresses the surgical care of infants and children. The intent of the CSV program is to match children’s specific providers and resources to the individual needs of every child needing surgical care. Learn more HERE.

Procedure Pass Pre-Op Status Board Improves Efficiency & Safety


The Quality Improvement Committee would like to express appreciation for everyone’s continued effort to implement the Procedure Pass process.  This tool was designed to improve safety and communication in the OR. With Procedure Pass, surgeons can now virtually communicate with the Perioperative team about readiness for the OR. Recent metrics show that adoption is expanding, yet as with all new processes there is room for improvement. These advances cannot work without your participation. Your feedback has already helped us identify useful changes. We sincerely appreciate your ongoing efforts to implement and your patience as we navigate the glitches together.

Marriott Business Council Leaders Tour Monroe Carell Jr. Children's Hospital at Vanderbilt ahead of Fundraising Efforts


Last year, funds benefitted Department of Pediatric Surgery, Vanderbilt Adaptive Sports and Greatest Needs Fund at Monroe Carell. They raise approximately $150,000 annually. 
On February 28, two new two new co-chairs of the Marriott Business Council joined Dr. Jeffrey Upperman to tour Monroe Carell.  Scott Seibert, Hotel Manager of Gaylord Opryland and Business Council Co-Chair, and Mark Giovetti, General Manager of The Inn at Opryland and Business Council Co-Chair will be leading fundraising efforts in the coming year.
Marriott International holds the distinction of being CMN Hospitals’ first corporate partner since 1983, surpassing 40 years of support. Marriott’s Nashville Business Council consists of all managed and franchised properties in the Middle Tennessee region. That is roughly 65 locations. Its associates and guests raise funds through hosting Cheers for Children, Tri My Best Triathlon, fundraising throughout Gaylord’s A Country Christmas, breakfast campaigns, and sponsoring the Holiday Music Special.

Supply Cost Reduction Program
Supply Cost Reduction work will continue in FY2023.
EPSi Cost Accounting Team has developed a Supply Cost Tableau dashboard to measure and track performance to expense goal and change in year over year costs .
The Supply Cost workgroup team was created to oversee this workstream consisting of Finance, Perioperative Services, and Surgeon Leadership. 
The team has identified 3 areas with opportunity for improvement:
1) Surgeon Preference Card Review
2) Reduce Supply Wastage
3) Utilization of CPT codes with greatest opportunity
