December 21 is Homeless Persons' Memorial Day: Planning Resources Available Online

On December 21, the first day of winter and the longest night of the year, we honor Homeless Persons’ Memorial Day (HPMD). Due to the COVID-19 pandemic, many communities amended or cancelled memorials last year; this year, we would like to encourage communities to come together to share in grief, love, and hope to move forward.

White Paper Explores "Data-Driven Solutions for Disrupting the Cycle of Homelessness"

The National Institute for Medical Respite Care recently hosted a webinar with leaders of Illumination Foundation who presented their co-authored white paper, "Data-Driven Solutions for Disrupting the Cycle of Homelessness: A Data-Driven, Analytical Approach to Illumination Foundation's Integrated Care Model for People Experiencing Homelessness."
Presenters shared data on Illumination Foundation's unique and effective integrated model for addressing the health inequities and Social Determinants of Health among those experiencing homelessness. Their work highlights how predictive and prescriptive data analytics have been used to improve the identification of the risks of the population Illumination Foundation serves, as well as to identify the most effective and efficient strategies to manage these risks, improve client outcomes, and reduce cost to the health care system.

Webinar: Strategies to Ensure No Child Sleeps Outside

Now is the time to reimagine your community’s crisis response system for families with children, using a racial justice and equity focus, including your approaches to sheltering families experiencing homelessness.
The Framework for an Equitable COVID-19 Homelessness Response project partners recently issued a brief, "Immediate and Flexible Crisis Options for Children and Families," to explore and support the expansion of crisis options, paired with strong housing exits, that can be replicated, adapted, and scaled up or down as needs and/or scale of resources change in each community, with the goal that no child sleeps outside.
A webinar on Thursday, December 9, will feature presentations by three of the report authors (Barbara Poppe, with Tiffany Haynes and Alexis Kramer), along with a panel of representatives from spotlight organizations that were selected and are featured in the report – Mahube-Otwa Community Action Partnership (MN), Cuyahoga County Office of Homeless Services (OH), and Mother Nation Seattle (WA).
The panel will be moderated by Joe Savage, U.S. Interagency Council on Homelessness, and Tabitha Blackwell, Funders Together to End Homelessness.

Learning Collaborative Opportunity: Understanding Non-Clinical and Case Management Staff Roles in Diabetes Prevention and Support

The Corporation for Supportive Housing (CSH) is offering a four-part learning collaborative for non-clinical and case management practices in care support and prevention of diabetes and diabetic complications for vulnerable populations. Community-based interventions that leverage both clinical and non-clinical strategies offer the opportunity to meet people where they are, manage existing cases, engage patients in their care plans, and even help prevent new diagnoses.
Beginning in February 2022, CSH will create an opportunity for health centers serving people experiencing homelessness to explore effective strategies to demonstrate that many promising diabetes prevention and self-management practices occur in settings other than the clinic. This learning collaborative will identify what housing case managers require to support continued diabetes prevention care and management and provide training to address those needs.
This learning collaborative will take place over the course of four 90-minute sessions. Facilitators will provide framework from a national perspective and facilitate interactions among participants to share local and organizational strategies, challenges, and successes. Health center staff with existing promising practices in the use of non-clinical services and housing case management will also be invited to share their perspective and best practices.
Applications to participate in this learning collaborative are due by Friday, January 14, 2022.
