Parental or guardian vaccine consent required
for students who are under age 18 years old

Dear Student,
You were recently informed that both the COVID booster and flu vaccines are being required for all students attending Tufts this year. All students must receive and provide proof of vaccination on or before December 2, 2022.
Our records indicate that you are currently under the age of 18. To meet the vaccination requirement, you must have your parent or guardian provide written consent to receive these vaccines. Your parent or guardian does not need to be present with you when you receive the vaccine.

For Influenza (Flu) vaccine
  • Please ask your parent or guardian to download and complete the flu vaccine consent form.
  • Have them return the completed form to you.
  • Contact Health Service at 617-627-3350 to schedule your flu vaccine appointment.
  • Bring the completed form with you when you go to get your vaccine.
Please understand that Health Service does not have enough doses of flu vaccine to immunize the entire student community, so students over the age of 18 are encouraged to use the University’s community clinics.
