Voluntary Delta Dental Insurance 2027

Dear Student,

Please read the Delta Dental Insurance Cover Letter regarding the voluntary Delta Dental PPO Plus Premier Plan that is being offered for the next calendar year. The new policy year will run January 1, 2027 through December 31, 2027. Also included are the rates, coverage details, and instructions to enroll in the plan.

Students are not required to enroll in the voluntary dental plan. The voluntary dental plan is separate from the health insurance requirement.

The deadline to enroll or re-enroll is December 1, 2026.

IMPORTANT ENROLLMENT INFORMATION:

CURRENTLY ENROLLED: Students currently participating in the plan must re-enroll for the new policy year by December 1, 2026. Coverage will NOT be renewed unless a new Enrollment form and Payment Form is received by EBPA by the deadline.
NEW ENROLLMENT: To enroll for the first time in the Delta Dental PPO Plus Premier Plan, complete the Enrollment Form and Payment Form and submit to EBPA by December 1, 2026.

Please note: You are NOT automatically enrolled in or billed for the dental plan because the plan is a voluntary plan. Students who enroll in coverage are electing to be enrolled the entire calendar year.

Mail required documentation and payment information to EBPA CL#218, P.O. Box 1387, Williston, VT 05495-1387.
Secure Portal Upload: Electronically transmitted through the EBPA Secure Document Submission Portal:

Select COBRA & Retiree/Direct Billing
Enter Tufts University – under Employer Name or Group Number
Document Type – ACH Forms
Select "Click here to submit claims, forms, or substantiation electronically"
Then select "COBRA and Retiree/Direct Billing" to upload your document(s).

When selecting a dentist, please make sure to select in the Delta Dental PPO network, https://www.deltadental.com/us/en/member/find-a-dentist.html. This is a PPO plan and can be used outside of Massachusetts.
