Homeless Persons' Memorial Day 2023: One Life, Too Many. Another Year, Too Long. Thursday, Dec. 21 at 2 p.m. CST

Every year, people in our communities die on the streets or in shelters without a home. Many will not have funerals or memorials. They deserve better.

Each Dec. 21 — the first day of winter and the longest night of the year — we gather for Homeless Persons' Memorial Day to memorialize and honor our family, friends, patients, clients, and community members who experienced the injustice of homelessness.

Please join the the National Consumer Advisory Board, the National Health Care for the Homeless Council, and the National Coalition for the Homeless on Dec. 21 at 2 p.m. CST for a virtual ceremony to remember those we've lost and renew our resolve to eradicate homelessness.

Spring Learning Collaborative Will Explore How to Incorporate the Cultural Safety Model

Cultural humility is an important step toward providing quality care tailored to health center patients, but it does not need to be the final step. Incorporating trauma-informed principles, providing space for patients to take an active role in decision-making and care, and looking for opportunities to learn more about a patient’s specific culture needs leads to positive patient encounters, increased patient satisfaction, and positive health outcomes. The Cultural Safety model is a means toward incorporating these ideals.

In spring 2024, NHCHC will facilitate a Learning Collaborative for Primary Care Associations and health centers to explore the Cultural Safety model and develop concrete steps toward training and implementing the model in their community context.

At the end of the Learning Collaborative:

Participants will be able to describe the Cultural Safety model.
Participants will identify at least one partner from their community.
Participants will gain the tools to develop regionally specific Cultural Safety training.
Participants will learn from the cohort’s experiences to develop a training guide.

A second iteration of the Learning Collaborative will be held in the next fiscal year and is intended to build on this current cycle. Applicants are encouraged to make plans to participate in the second-year process. Please join us!

How Electronic Health Records Can be Leveraged to Streamline Social Needs Screening

Is your health center planning, implementing, or revamping a social needs screening program?

Join NHCHC and the HITEQ Center on Feb. 6 at 1 p.m. CST to learn how screening for housing status and other Social Determinants of Health (SDoH) measures can be introduced or better integrated into health center clinical workflows.

Presenters will share guidance on implementing and systematizing social needs data collection in Electronic Health Records, followed by a panel of expert health center representatives who will speak about their programs’ journeys with social needs screening programs. Participants will have the opportunity to ask questions, share successes, or discuss specific challenges they face regarding social needs screening.
