Team Spotlight: Kahle Tobias
Kahle (“Kay-Lee”) is a quality improvement coach in Partners For Kids’ West Region. As a nurse, Kahle earned a Certificate in Pediatric Nursing and assisted in the opening of adult COVID-19 units. She ultimately has found her passion in pediatrics and currently works with South Dayton Pediatrics, Cornerstone Pediatrics, Pediatric Associates of Dayton and Springboro Pediatrics on population health meetings and practice specific projects (i.e., Well Child Visit, Asthma, Constipation, Potentially Avoidable ED Visits, Outreach). Kahle also participates in various workgroups at the organizational and regional levels, including the constipation collaborative, preventative care workgroup and behavioral health workgroup, focused on ADHD. Kahle is a life-long learner and pursuing her master’s in nursing informatics

Registration Open for Annual Administrative Retreat
Partners For Kids is hosting the 6th annual retreat for practice administrators and office managers, “How a Practice Administrator Can Impact a Practice’s Success.” This virtual event will be held on Friday, June 21. Join us as we discuss community resources and social determinants of health. Our morning session will walk through the Partners For Kids’ Community Resource Library and FindHelp.org, two databases that enable practices to connect patients with resources in multiple languages.

Partners For Kids’ Care Coordination Awarded NCQA Accreditation
Partners For Kids’ Care Coordination was awarded a 3-year accreditation status upon completion of the NCQA (National Committee for Quality Assurance) Complex Case Management accreditation survey. This is the highest category awarded by NCQA for service and quality, that demonstrates strong performance of rigorous requirements for case management.

Update on Partners For Kids’ Provider Portal Access
The Partners For Kids’ Provider Portal has a wealth of data and can be a valuable tool for practices. The Managed Care Organization requires Partners For Kids to audit who is accessing the data and how often. If you have not accessed the provider portal within the last 12 months, your access may be revoked. If you are having difficulty logging in or are interested in gaining access to the provider portal, please contact PFKProRelations@ChildrensDayton.org.

Reminder: 2024 Partners For Kids Primary Care Network Requirement
Every Partners For Kids credentialed primary care provider must participate in at least one activity that satisfies the Partners For Kids primary care network requirement by December 31, to ensure eligibility for 2024 incentive bonuses.

Oral Health Toolkit Now Available
Partners For Kids has developed an oral health toolkit to assist primary care practices and providers when addressing the oral health needs of their patients. The toolkit highlights a curated list of educational materials, resources for patients and providers, and helpful links. The information housed in this toolkit will be updated as needed based on current clinical guidelines, recommendations, and data.

Posters Added to the Healthy Children Toolkit
The Healthy Children Toolkit now includes patient and family-facing posters that describe the importance of well-care visits, and the differences between well-care visits and sports physicals. The toolkit also includes best practice resources to assist you and your staff in messaging the importance and specifics of well checks to families.

Primary Care Provider (PCP) Change Request Form Now Available on the Provider Portal
The Partners For Kids Provider Portal’s mismatched attribution report shows a list of patients who were seen for a well-care visit at your practice, but who are not attributed to the provider completing the visit. This report now includes a link to the PCP Change Request Form. If a family wants to change their attributed provider, they can complete the PCP Change Request Form and send the updated form to the patient’s assigned Managed Care Plan.
