Advancing 4Ms Care of Older Adults and Care Partners Living
with Dementia:
The Kansas Dementia Care Network and Alzheimer’s State Plan

This session will explore how the Kansas Dementia Care Network and Alzheimer’s State Strategic Plan align with the Age-Friendly Health Systems 4Ms Framework (What Matters, Medication, Mentation, and Mobility) to strengthen prevention, awareness, and support for individuals living with dementia and their care partners.
Designed for physicians, advanced practice providers, nurses, social workers, pharmacists, pharmacy technicians, and other health professionals serving older adults in rural and underserved communities.
