Whether you were recently diagnosed, have had diabetes for years, or have learned you’re at risk, this guided tour through diabetes self-care is for you. The event features three activities (attend any or all):

Nurse visit:
This meeting will ensure your medical care is up to date (diabetes diagnosis and RSVP required)*
|

Nutrition session:
Learn the truth about sugar, how to read food labels, blood sugar monitoring tips, and try some tasty recipes
|
| Mini fitness sessions:
Sample a strength training class, a yoga class, or both (beginner friendly)
|
|
|
* Nurse visits are reserved for those diagnosed with diabetes. Nutrition and fitness sessions are open for all.

Nurse visit: Catch up on care |
This 30-minute session is limited to those already diagnosed with diabetes. You'll meet with an MNPS Health Care Centers nurse practitioner to ensure you’re up to date on care.

- A1c check
- Urine protein testing
- Eye exam
- Foot exam
- Vaccinations

| - A summary of your visit to share with your primary care provider
- Recommendations for follow-up care*
|
|
|
Appointments scheduled from 3:00 to 6:30 p.m.
|
* Staff cannot provide medication changes or refills at the fair. The fair does not replace regular follow-up visits with your primary care provider.

Beginner yoga with Jessica: 4:30 p.m.
