Health and Consent Form Tips
|
As you prepare for camp, the earlier health and consent forms are completed the better! We recommend sending out your trip’s health form links to parents at least three weeks before your departure date. This helps reduce the number of last-minute details for your area to manage. Here are a few tips to keep in mind:
|
-
A completed Health and Consent Form is required for every camper and leader, for each trip they attend.
- If your camp trip requires additional documentation (immunization records, medical signatures, etc.) we recommend sending the health form link out as many as four or more weeks prior to give parents enough time to gather the necessary information.
-
Be sure to verify that the camper’s name is listed correctly on the Arrival Form (opposed to the parent's name) prior to sending the health form link, as the health form’s name field is not editable. If the camper’s name needs to be updated on the arrival form, please reach out to the Connections Team.
- Parents/participants must enter the date of birth on file in YL Connect prior to completing the health form. If the listed date of birth is incorrect, see how to change a camper’s or a leader's date of birth listed on their health form.

Camp-in-Action: Video Scrapbook
|
Want to see camp in action? Our awesome video interns are creating amazing videos that showcase the best moments of camp each week. Head over to our video scrapbook website, and find your camp week. Also, it’s the perfect way for parents to catch a glimpse of the Young Life camp experience so share the link with them!
Videos will be uploaded as interns have availability, so be patient if you haven’t seen the video you are looking for!

Thanks for walking with kids. Day Zero will take a summer camping break in July, but we’ll be back in August!
Happy summer camping!
