Email Exhibit 15T to Michael Thibodeau and call him at 515-452-0436, as early as possible, but no later than 15 days prior to February 19, 2025, to discuss the confidentiality request. Any IFA-approved redacted exhibit must be uploaded to the application.

Qualified Service Provider Requests |
Qualified Service Provider approval requests must be submitted through the Application between February 1-12, 2025, and must include Exhibit 1HSA. Exhibit 1HSA must have a copy of the qualified service provider’s most recent independent audit within the past 2 fiscal years attached.
