Completion and submission of an Annual Student Health Update form is required for all 2026-2027 PK-8 students.
Our school nurse uses this information to ensure our records on and plans for all students are current and appropriate. The update form also allows parents to authorize the school to seek emergency medical treatment for children if necessary and to authorize the school to follow its asthma/anaphylaxis protocol for a student if necessary. Such authorizations are essential in order for school staff to act appropriately in critical situations.
Annual Student Health Update forms will be completed and submitted online for the 2026-2027 school year using Ecollect (the same platform recently used for registration).
To complete forms for your children, log into your parent PowerSchool account (link provided below for that login page). Then, for EACH student select the Forms option in the left navigation pane in order to access the Annual Student Health Update form.
For students starting at SVdP for the first time next year, parents will see that registration forms previously completed are now showing as "Not Started." Parents can ignore those forms (we have the data) and just complete and submit the Annual Student Health Update form.
NOTE: You will need to select each of your children individually in turn and then complete and submit the form for each.
